Horizon Hospice and Palliative Care

Who We Are

Hospice Care Horizon offers a holistic approach to hospice care. Our interdisciplinary team supports the primary caregiver in addressing the physical, psycho-social and spiritual needs of our patients.

What We Do

Our volunteers are compassionate individuals who recognize that their presence can make a big difference in the lives of our patients. Volunteers desiring to serve patients directly attend an orientation class. We also require, provide, and pay for: background checks, TB tests, and driving record checks for all our volunteers. We ask our volunteers to commit to one year of volunteer service consisting of a minimum of one hour per week. Some of the things our patient volunteers do include: Offer companionship Be a listening ear Share a game or hobby Write a dictated letter Run errands Read to a patient Sit with a patient to give the caregiver a chance to run some errands Give a patient a simple hand massage Paint fingernails Provide wheelchair tours for some fresh air Pet therapy Our non-patient volunteers help in the following ways: Contributing Quilts, Afghans, Specialty projects like fidget blankets, bone pillows, contracture pillows and clothing protectors Contributing Handmade Cards Office-Mailings, sorting, organizing, events Delivering Quilts and Afghans to patients for special events Recruiting events and managing a booth at a community event
