Planning and Management Council (PMC) Members

  • Collaborate with staff to create coordinated and comprehensive service delivery systems
  • Provide planning, program development and administration, public information, advocacy, and direct service
  • Emphasize that the above functions and services are targeted toward those with greatest social, economic and health needs and on culturally diverse individuals
  • Promote a long term care system through integrating acute and chronic care services
  • Create innovative outreach and information mechanisms to reach isolated individuals

Application Process

Once interested candidates complete and return the volunteer application form, they will be invited to interview with members of the Council.  Aging & Long Term Care Governing Board approval is required before membership begins.

Interested candidates can also attend a PMC meeting to see the Council in action.
